Comprehensive Benefits

All eligible employees have a choice between two (2) Blue Cross/Blue Shield PPO plans that provide direct access to any participating provider for covered benefits, with NO referrals required. Liberty offers a traditional co-pay plan as well as a high deductible plan. To help offset the deductible, Liberty contributes money into a health savings account for participating employees in the high deductible plan. Employees are able to cover themselves or opt to cover their spouse and/or dependent children up to age 26.

Prescriptions can be filled at more than 100,000 pharmacies nationwide, or acquired through a mail order program that allows a 90 day supply.

All eligible employees have a choice between two Met Life Dental PPO plans.  Employees are able to cover themselves or opt to cover their spouse and/or dependent children up to age 26.

All eligible employees have the option to choose our voluntary vision program through Vision Benefits of America.  Employees are able to cover themselves or opt to cover their spouse and/or dependent children up to age 26.

All eligible employees have the option to purchase supplemental illness and accident coverage that becomes effective if and when certain ailments are diagnosed or covered accidents occur.

Basic life and ADD insurance is FREE to all eligible employees at one times the employee’s salary up to $50,000. Additional Life and ADD is optional.

Long-term disability coverage, which provides valuable income protection should an individual become seriously ill or injured, is FREE to all eligible employees.

Liberty gives its employees the option to purchase short-term disability insurance.

All eligible employees have the option to choose additional life insurance for themselves, their spouse and/or their dependent children.

Flexible spending accounts give employees the opportunity to save money for non-reimbursable health and child care expenses on a pre-tax basis. Liberty offers the opportunity to enroll in a Health Care Flexible Spending Account and/or Dependent Care Flexible Spending Account to all eligible employees.

Liberty employees are invited to access Health Advocate, which provides guidance and information via telephone by experienced healthcare professionals who can assist with:

• Scheduling physician appointments, testing and second opinions
• Answering questions about test results, treatments and medications
• Locating the right treatment facilities
• Explaining health coverage policies and mediate coverage denials
• Pricing out medical services

Liberty provides NO-COST, 24/7 access to this service for its employees and their families including parents and parents-in-law. A recent article in the Chicago Tribune shed light on the value of this benefit when it pointed out that a person paying out of their own pocket for this type of service can expect to pay $75-$300 per hour.

Liberty gives its employees access to a comprehensive Employee Assistance Program (EAP) that provides NO-COST, confidential telephone consultation with licensed clinicians who can provide immediate crisis resolution, as well as information and referrals to appropriate counseling and support services for such issues as:

• Marriage, relationship and family problems
• Problems at work
• Changes in mood
• Legal and financial issues
• Stress and anxiety
• Alcohol and drug dependency
• Identity theft
• Health and wellness concerns
• Grief counseling
• Locating child care, elder care, and/or pet care services
• Preparation of legal documents including wills, legal wills, power of attorney forms, and HIPPA authorization forms

Liberty Wellness Works is Liberty’s award-winning employee health and wellness program that helps employees not only succeed in fitness pursuits, but also better address nutrition, stress and financial wellness. Liberty Lutheran invites all eligible employees to participate in a 403b retirement plan that, in addition to an employee’s contributions, is supplemented by Liberty with a company match and periodic discretionary contributions.

Liberty encourages and supports employees in their personal and professional development. Eligible employees may apply for tuition assistance of up to $2,500 per year to pursue an advanced degree, an Associate or Bachelor degree or a professional certification.

After seven (7) years of continuous service at Liberty, all full-time employees are eligible for a paid six (6) week sabbatical.

Liberty recognizes the following eight (8) holidays:

New Year’s Day, Martin Luther King Day, President’s Day OR Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day.

The amount of paid Annual Leave time you will receive depends on your length of service with Liberty and its Family of Services. Annual leave is earned as follows:

• Date of hire to 2^nd Anniversary – 15 days
• 2^nd to 5^th Anniversary – 20 days
• 5^th to 10^th Anniversary – 23 days
• 10^th to 15^th Anniversary – 26 days
• After 15^th Anniversary – 30 days
